Add files to an entry
POST/api/v1/entries/:id/files
Any entry, not only a note: a consultation carries the clinic's report and a weighing carries the photograph of the scale. See decision 0035.
Separate from creating one because that is how somebody uses it: a note is started with what they had to hand and grown as more turns up.
